Here's a JPEG file, really close to what you are looking for. My software won't allow me to re-size fonts incrementally, so it's just a bit off, but it may look better in a larger format.

Most any sign-shop should be able to make a plate from this. If not, I can supply you with another graphics format they can use, just let me know.
