Mounting the winch under the floor would be nice. What type of pulley did they use? Is it just a roller? I would think a pulley mounted close to the winch wouldn't allow the cable to go back and forth on the drum as it winds up.

As far as driving the car while winching. I pull my car onto the tailgate and then hook the winch up. I don't have a remote and because of the ATV winch I use it's just mounted to the D ring on the floor with a clevis. I just stay by the winch and pull it in, never had to steer it.
I would like to permanently mount a winch along the wall and use a pulley on the D ring. I don't know how that would work out with a synthetic rope instead of a cable.

I have my winch on top of the floor, behind a lower door under the bench.
Only use it to load the car, if it is not drivable.

The guy I quoted in my post was trying to describe something his buddy had built and used the best language he could. The "pulley with ears" he describes is probably a snatch block, a pulley used for winching and recovery. It rotates open to load the cable and closes to hook to the car with the cable trapped in it. It also multiplies pulling power if used right. Hook the snatch block to your tow point and run the cable back to the head to the trailer in a loop. The cable is anchored at two points now and has twice the pull power. Takes twice as long to pull, though.
The first snatch block probably bolted to the frame member closest to the pivot point. The second could be bolted to a bracket at the head of the bed low and welded to the rails. A slot would need to be cut in the deck for the cable to come up, but not too big or long. As for the cable following the winch drum, there would be around 8 feet or more to pulley one so it should track OK.
Here's a snatch block.

Mid West Race Cabinets offers a kit to install the winch under the floor.
