I have two "Henry's" spool and axle12 bolt rear ends and that looks familiar to me. I haven't looked at mine in a long long time. I also have a Summers Bros. Dana 60 and if I recall they look more like what you see today and the Henry's stuff was a little different. Both 12 bolts also have Olds bearing ends installed as was a common mod back at that time. I think Henry's was out of California. I'm not saying that is what you have but it sure brings back those recollections.

Yes, Henry's Machine Shop was located in Buena Park CA not too far from
Knotts Berry Farm, long gone now. But I remember it from the 70's.
