About 6" long piece of angle aluminum stock with 2 holes through the web of the 12 bolt to mount it. A slight twist put into it with and pair of adjustable wrenches.

This is on a Dana but the same idea I applied on my 12 bolt. Just made the aluminum piece a little cleaner. That one we made in a hurry and is the only pic I have. Then a small angle bolted on for the sensor.

Re: GM 12 Bolt - Racepak Drive Line Sensor Bracket
I've made a couple, one for my son's and for my own car, both for 12-bolts. Similar to what Rich B. above says, I used a piece of 1/4" thick x 1" wide 90° angle aluminum x about 6" long. Drilled a pair of 1/4" holes through the webs for mounting. Mine are certainly not CNC quality but I think as long as the sensor winds up .050"-.100" from the split collar, you're alright.
